Entry Visa Copy (with Kuwait entry stamp)
Passport Copy

Attested Medical Qualifications (Degree/Diploma) – attested by Kuwait Embassy
Attested Experience Certificates – attested by Kuwait Embassy
Medical License/Registration Certificate
Medical Fitness Certificate
Police Clearance Certificate – attested by Kuwait Embassy
Updated Curriculum Vitae (C.V)
