📋 Required Documents for Locum Contract Submission – Step-by-Step Guide

Please prepare and arrange your documents in two separate folders as outlined below:

Folder 1: Locum Contract Signing Documents

Bring copies of the following documents, arranged in the same order:

Entry Visa copy (with Kuwait airport entry stamp)
Passport copy (with Kuwait airport entry stamp)
Professional Qualification certificates (Degree/Diploma) – attested by the Kuwait Embassy
Experience certificates – attested by the Kuwait Embassy
Medical License / Registration Certificate copy
Medical Fitness Certificate copy
Police Clearance Certificate – attested by the Kuwait Embassy
Updated Curriculum Vitae (CV)

Folder 2: Additional Documents

Prepare the following sets. Write your MTON code on all documents.

Set 1 (For Medical):

Entry Visa copy
Passport copy
2 photographs (size 6×4 cm, blue background)
Write your Kuwait phone number and marital status on the Entry Visa copy

Set 2:

Color copy of Passport (with Kuwait airport entry stamp on a single page)
Color copy of Entry Visa (with Kuwait airport entry stamp)

Set 3:

Entry Visa copy
Passport copy

Important Notes:

Clearly write your MTON code on all documents.
If you wish to apply for an accommodation allowance, bring an additional copy of your Passport and Entry Visa.

✍️ Locum Contract Instructions:

Sign on the left-hand side of the Locum Contract.
Return all 3 pages of the signed contract along with the documents listed above.
Carefully verify the following details in your Locum Contract:
- Date of Joining
- Full Name (exactly as in your passport)
- Passport Number
- Locum Salary

If there are any errors or mismatches in these details, do not sign the contract. Instead, notify us immediately so we can correct and reissue the updated contract for your signature.
